How can I become an AIM patient?

Fill out the New Patient Appointment Request Form and our staff will be happy to help you join AIM!

Please read the following information prior to scheduling your first appointment:


-Please leave any voice messages related to new patient appointments on Line # 3 if we are unable to answer your call immediately.

-We ask that you allow us at least one week advance notice to the date you request for your first visit.

-The first visit at our office is considered a "meet-and-greet" for the patient and provider.

-Don't forget to bring all medications, important medical concerns, etc with you to your first appointment.

-Our practice may not treat, diagnose, or prescribe on the day of your establishing visit.

-Physicals, sick patient appointments, etc. will likely be scheduled after your initial visit.

Please complete the following forms prior to your first visit at our office:


Medical Record Release Form
This will allow us to request that any of your medical records be sent to our office if they are currently with another provider. This form requires your handwritten signature and therefore cannot be signed electronically through our Patient Portal.


Patient Registration and Health History Forms

This information is available for online completion through our Patient Portal and updates automatically into our electronic medical record system. We highly recommend using the Portal to avoid filling out paperwork in the waiting room. However, you may also complete and submit these forms to us by emailing, faxing, or dropping them off during our office hours.

Can I complete the registration process online?

For your convenience, you may now update your health history and electronically sign our registration forms. If you choose to complete your information online, it will automatically update into our system. Please call our office to obtain access to the portal.


What types of insurance does AIM accept?

We accept most insurance plans and self pay patients are welcome.

Co-pays are expected during your check-in at the front desk. If your insurance does not specify a specific amount for your co-pay, we will collect $25 to go towards the date of service. We accept cash, checks, VISA, MasterCard and Discover. For your convenience, payments can now be made over the phone, through our Patient Portal or by mail.


What do I need to do for my first appointment?

Valid insurance card (if applicable)

  • Photo identification

  • Co-pays required by your insurance (will be collected at check-in)

  • All medications (in the bottles) you are currently taking including prescription, over-the-counter, and/or supplements
